Change of Major/Minor

Instructions for Independent Study Form:

  1. Download this form and open with Adobe Acrobat.
  2. The form is set up for typing and also digital signatures.
  3. The student types form in consultation with the appropriate faculty member.
  4. Student signs and sends via email to the faculty member sponsoring the independent study.

    Instructions for Transient Study Form:

    1. Download this form and open with Adobe Acrobat.
    2. This is set up for typing, but not digital signatures since it goes beyond the department for approval and processing.
    3. The student types/fills out in consultation with the appropriate faculty members, getting the approvals for each class.
    4. The student prints the form, signs, and submits a paper copy to Teena Power in the Department Office so she can obtain department approval and then process accordingly.

    Instructions for Undergraduate Internship Form:

    1. Download this form and open with Adobe Acrobat.
    2. The form is set up for typing and also digital signatures.
    3. The student secures an internship opportunity with an agency, business, or non-profit. Opportunities are sent out regularly via email to our department majors when they are presented to the department. Sometimes faculty are aware of internships, but not always.
    4. The student types form in consultation with the appropriate faculty member, along with the place of the internship and contact information.
    5. Student digitally signs and sends via email to the faculty member sponsoring the internship study.

    2. The form is set up for typing and also digital signatures.
